Prognosis of the Small Cell Carcinoma of the Uterine Cervix: A comparative Study

Abstract
The clinical courses of 19 patients with small cell carcinoma of the uterine cervix(SCC) were reviewed and compared with that of 224 cases of squamous large cell carcinoma(LCC). We observed the following differences between the two:
1) SCC patients had a higher incidence of lymphovascular space invasion and lymph node metastasis. Especially, lymphovascular space invasion was significantly(P<0.05) more common in SCC953.3%) than in LCC(23.8%).
2) Recurrence rate was significantly(P<0.05) higher in SCC(36.8%) than in LCC(18.3%).
3) The 54-months disease free survival rate for patients with SCC in all stages was 46.1% versus 57.3% for matched patients with LCC in all stages. This difference was not statistically significant(P>0.05).
4) The 54-months disease free survival rate for patients with stage¥° SCC was 60.0% versus 82.9% for matched patients with¥° LCC in all stages. This difference was not statistically significant(P>0.05).
5) The 26-months disease free survival rate for patients with stage¥±SCC was 28.6% versus 74.1% for matched patients with stage¥±LCC. This difference was statistically significant(P<0.05).
6) The 33-months disease free survival rate in stage¥°,¥±SCC only surgically treated was 72.0% versus 95.1% for matched patients with stage¥°,¥±LCC only surgically treated. This difference was statistically significant(P<0.05).
Clearly, small cell cancers of the uterine cervix become a systemic disease rather early in the disease process and should be treated accordingly. Small cell cancers have not responded well to traditionally adequate and it is apparent that some
forms of
primary adjuvent chemotherapy is indicated in these patients.
